How do I get a New Leisure Access Card?

How do I get my First Photo Leisure Access Card?

If you are receiving Ministry Income Assistance:

• Bring in the complete application form to a photo card location
• You and your family's information, if applicable, will be entered in the Photo ID system
• Your photograph is taken
• Your card is produced
• Other family members can come in at any time to have their picture taken

If your application is being processed by the LAC Program office:

• You will be notified if you qualify
• Once notified, go to a photo card location (listed below) to have your photo taken
• Your information will already be entered in the system
• Each member of your family 6 years and older, will need to have their photo taken
• Your photograph is taken
• Your card is produced

To replace lost or stolen Leisure Access Card

• Report your lost or stolen card to a photo card location or to the LAC Program office
• A replacement card will be issued
