Report fraud and misconduct by City staff

This online form provides members of the public with a confidential means to report incidents of serious misconduct, fraud, waste or wrongdoing to the Office of the Auditor General for the City of Vancouver.

Use this form to report matters such as:

  • Fraud
    • Deception intended to result in financial or personal gain including but not limited to:
      • Bribes
      • Misappropriating funds
      • Breach of trust
      • Theft
      • Misuse of City funds or resources
      • Theft of City assets
      • Manipulation or falsification of City accounting or audit data
  • Waste 
    • Mismanagement of City resources or assets in a willful, intentional, or negligent manner
  • Serious wrongdoing
    • Serious financial act or omission that would constitute an offense under the enactment of BC or Canada
    • Serious misuse or mismanagement of public funds or assets, such as fraud or waste
    • Conflict of interest
    • Instructing or encouraging another employee to commit serious wrongdoing
    • Deliberating concealing information related to wrongdoing

Writing your report

Be as specific as possible when describing your allegation. Include locations, times, dates, and who was involved, including people and departments. Don't worry about writing style or format.

Your contact information and confidentiality

Your contact information is not required to report an allegation. However, there may be cases where we are unable to fully investigate an issue without communicating with you.

If you choose to provide your contact information, it will only be used to follow up with you on your report, if needed.

Your name will not be disclosed unless required by law.

Your personal information is collected by the Office of the Auditor General for the City of Vancouver under the authority of section 26(c) of the Freedom of Information and Protection of Privacy Act (FIPPA). It is collected for the purpose of investigating allegations of serious wrongdoing as defined by the City of Vancouver’s Whistleblower Policy COUN-010 (150 KB).
