Community Services GrantsINSTRUCTIONS FOR APPLICANTSApplications for the 2012 Community Services Grants are now available. The deadline for applications is FRIDAY, OCTOBER 21, 2011. Please note that applications should be sent to the Social Policy Division's new offices located at: 111 West Hastings Street (5th floor reception). The Information Sheets available at the bottom of this page include information about the three grants categories, the Outcomes Measurement Framework being used in the applications, and, in the case of the Direct Social Services Grants stream, the Priorities List associated with funding allocations. Instructions for applying for the 2012 Community Services Grants are as follows: 1. Before completing your application form, read the appropriate Information Sheet attached to each. (Note that each of the three grants streams has a separate set of instructions). Call Social Policy staff at 604.829.2007 if you have any questions. If you are a first time applicant, you should call a Social Planner to discuss your proposal before submitting an application. 2. There are three distinct categories of Community Services Grants. Each has its own application form. Please ensure that you are applying for the appropriate type of grant. If you are unsure, please call Social Policy staff. If you are applying for more than one grant, complete a separate application form for each one.
3. Application forms are distributed in an electronic format (Microsoft Word). If you are unable to fill out the application in this format, please contact Social Policy staff at 604.829.2007. 4. Please supply concise and up-to-date information directly on the application form. Include all the relevant documents requested, and limit further attachments to a few pages. Since page 1 will be copied and distributed to City Council, it should be completed in full, with no attachments or references to attachments. Faxes and/or blue ink cannot be accepted. 5. When you have completed the application, e-mail an electronic copy to socialpolicygrants@vancouver.ca. Give the application file a name that indicates which organization it’s coming from. We also require 2 printed, signed copies of the completed application. Mail or deliver these to Social Policy, along with the following documents listed below:
Please note that we may require additional documentation when your application is reviewed by City staff. 6. Social Policy staff will meet with each eligible grant applicant in person or by phone to go over their application. At least one Board member must be in attendance at the face-to-face meeting. Please complete the calendar form, indicating when you can be available for this meeting. 7. Return 2 printed, signed copies of the completed hard-copy application form and attachments to the new offices of the Social Policy Division at the address below, in the historic Woodwards building. Applications can be received by mail, by courier, or by hand delivery to the 5th floor reception desk.
OUTCOMES MEASUREMENT TRAINING MATERIALTraining materials from the Outcomes Measurement workshops are also available for download. These materials may be useful when completing the Outcomes Measurement tables that are included with each application.
